Heal Atresia in the Omphalocele




Abstract
Atresia of small intestine is among the most common causes of intestinal obstruction in the newborn. During the past 20 years, the early recognition and adequate treatment f have undergone marked reduction in morbidity and mortality. The pathogenesis, however, is still not fully understood, but many authors believe that intrauterine mesenteric vascular catastrophe,: especially during period, of physiologic umbilical hernia, may be etiologic factor.
This is a case of unique combination of ileal atresia and omphalocele with ateretic segment in the omphalocele.
A 3-day-old male patient was admitted due to bulged ass at the base of umbilical cord, bilious vemiting and progressive abdominal distension. Plain rentgenogram of abdomen and colon study revealed complete small bowel obstruction at the site of omphalocele, with micro colon. On exploration, complete agenesis of small bowel segment with mesenteric defect was identified in the omphalocele. Segmental, resection and, end to end anastomosis was performed and omphalocele was repaired with difficulties. Postoperative course was uneventful, and patient was discharged on 7th postoperative day.
